糖尿病患者鞋内压力监测系统的预警阈值测试

Early Warning Threshold Test of the Pressure Monitoring System in the Shoes of Diabetic Patients

摘要: 探究与II型Wagner0级糖尿病足溃疡风险增加相关的足底峰值压力、压力不适和疼痛阈值,为监测糖尿病患者足底压力的鞋内压力监测系统设定压力预警阈值提供数据支持。使用Footscan足底压力平板采集347名II型Wagner0级糖尿病患者足底10个分区的峰值压力,使用压力测试仪采集30名健康人足底9个测试点,在测试速度0.5、1.0和2.0 mm/s下进行压力不适和疼痛阈值测试。结果发现,II型Wagner0级糖尿病患者左、右足全足的足底峰值压力均表现为男性大于女性,足底主要受力区域为第2、3、4跖骨和足跟。健康人足底压力不适和疼痛阈值具有性别差异(P<0.05)的测试点中,第5跖骨、足弓外侧和后跟中心均表现为男性大于女性,第3跖骨均表现为男性小于女性;男、女性足底压力不适和疼痛阈值整体趋势表现为后跟中心>足弓外侧>跖骨>足弓内侧>大脚趾;在同一测试点下,测试速度越快,测得的压力不适和疼痛阈值越高。综上所述,II型Wagner0级糖尿病患者鞋内压力监测系统的压力预警阈值设定,需根据患者性别和足底区域进行具体调整和决定,建议将糖尿病患者的足底峰值压力、健康人群的足底压力不适阈值和压力疼痛阈值分别设定为压力监测系统中的III级(一般)、II级(较重)和I级(严重)预警值。

Abstract: This work explored the threshold values of peak plantar pressure, pressure discomfort and pain associated with the increased risk of diabetic foot ulcer type II Wagner0, in order to provide data support for setting a pressure warning threshold for the in-shoe pressure monitoring system that can detect the plantar pressure of diabetic patients. A Footscan plantar pressure plate was used to collect the peak pressure in 10 zones of the plantar of 347 patients with type II Wagner0 diabetes mellitus, and a pressure tester was used to collect 9 test points of the plantar of 30 healthy people. The pressure discomfort and pain threshold were tested at the test speeds of 0.5, 1.0 and 2.0 mm/s, respectively. The results showed that the peak plantar pressure in the left and right foot of type II Wagner0 diabetes mellitus was greater in males than in females, and the main stress areas of the plantar were the 2nd, 3rd, 4th metatarsal bone and heel. In the test points of plantar pressure discomfort and pain threshold of healthy people with gender differences (P < 0.05), the fifth metatarsal bone, lateral arch and heel center were more male than female, and the third metatarsal bone were less male than female. The overall trend of plantar pressure discomfort and pain threshold in men and women was center heel > lateral arch > metatarsal > medial arch > big toe. At the same test point, the faster the test speed, the higher the pressure discomfort and pain threshold measured. To sum up, the pressure warning threshold setting of the pressure monitoring system in shoes for type II Wagner0 diabetes patients needs to be adjusted and decided according to the patient's gender and planta region. It is suggested that the peak plantar pressure of diabetic patients, the threshold of plantar pressure discomfort and the threshold of pressure pain of healthy people should be set as level III (general), level II (severe) and level I (severe) warning values in the pressure monitoring system, respectively.
