鞋底对老年人负重行走稳定性的影响

Influence of Shoe Sole on the Stability of the Elderly Walking with Load

摘要: 研究不同硬度和厚度的乙烯-酯酸乙烯共聚物(EVA)鞋底对60周岁以上老年人正常行走以及负重情况下步态稳定性的影响。实验用鞋为定制,尺寸为中国码38码,鞋帮面材料为飞织面料,鞋底材料为EVA,鞋底硬度分为48、53和58邵尔A三档,厚度分为2 cm和3 cm二档,共六双,所有鞋子除鞋底的硬度和厚度不同其余均相同。负重等级分为无负重和体重的15%。使用步态分析仪和三维测力台测定与稳定性相关的步态时空参数和动力学参数。结果表明,在无负重和15%体重负重情况下,均是硬度中等或比较硬(53邵尔A)且厚度较薄(2 cm)的鞋底的鞋子有利于保持老年人行走时的步态稳定性,而较软且较厚的鞋底不利于稳定性;15%体重的负重会降低行走稳定性,并且负重时不同硬度和厚度级别的鞋底之间的稳定性差距变大,老年人在负重状态下着鞋行走更应注重鞋底的选择,尽量选择鞋底硬度中等或较硬、厚度较薄的鞋子,以提升行走稳定性,降低跌倒风险。

Abstract: This paper studies the effect of different hardness and thickness of ethylene vingl acetate copolymer(EVA) soles on gait stability of the elderly over 60 years old when walking normally and carrying weight. The experimental shoes are customized in Chinese size 38. The hardness is divided into three grades including 48 Shore A, 53 Shore A and 58 shore a, and the thickness is divided into two grades including 2 cm and 3 cm. There are six pairs in total and all shoes are the same except for the hardness and thickness of the sole. The weight-bearing grade is divided into no-load and 15% of body weight. Gait spatio-temporal parameters and dynamic parameters related to stability were measured using a gait analyzer and a three-dimensional dynamometer. The results show that shoes with medium or relatively hard (53 Shore A) and thin (2 cm) soles are beneficial for maintaining the gait stability of the elderly when walking with or without load, while soft and thick soles are not conducive to stability. Weight bearing reduces walking stability, and the stability gap between soles of different hardness and thickness levels becomes larger when carrying weight. The elderly should pay more attention to the choice of soles when walking with shoes with loads, and try to choose shoes with medium or hard soles and thin soles for improved walking stability and reduced risk of falls.
